What to Expect When You Join Memory Care
Lincoln Glen Memory Care -- providing care for seniors in a loving
family-like environment.
Located in the heart of Willow Glen, Lincoln Glen
Memory Care is situated in a quiet and central residential
neighborhood. The grounds are maintained in a park-like atmosphere and are close
to shopping, churches and hospitals.
There are no buy-in fees or leases and a package of services is included that
will help to enable a memory care resident to meet new friends, live
comfortably, be satisfied and remain active. Additional and optional care
services are available for those who need them for an additional charge.
The following BASIC services are included in the monthly charges for all
Memory Care residents:
- 24 hour well-trained staff supervision and emergency response in a secure
environment.
- Care supervised by a Registered Nurse (Health Services Coordinator)
who directs and trains all staff members.
- 3 nutritious meals daily in the comfortable and easily accessible Dining Area
(including MD prescribed diets as required).
- 3 snacks daily (AM, afternoon and evening).
- Weekly laundry services and free use of first floor laundry facilities.
- Weekly complete housekeeping and linen service.
- All utilities (except phone)
- Bedside care for up to 3 days for temporary/minor illness per month.
- Monitoring, ordering, storage and timely distribution of MD ordered
medications (see note below).
- Observation and supervision for changes in physical, mental, emotional and
social functioning (see note below).
- 24 hour monitoring and assistance with the activities of daily living and
personal needs according to the Quantitative Assessment of Resident Care Needs
with an assessment score up to 6 in any combination of areas.
Note: Medications, observation and supervision are provided in accordance with
the Quantitative Assessment of Resident Care Needs with an assessment score up
to 6 in any combination of areas as part of Basic Services. Scores above 6 will
result in additional charges. Information on Assessment Scoring available by
calling and speaking to the Administrator or Health Services Coordinator.
Additional & Optional Assistance & Care Services
Quantitative Assessment of Resident Care Needs (Scores of 7 or more).
As residents age in place, additional services or assistance may become
necessary. Many necessary additional services are available on an as-needed
basis. This allows those residents who require additional services to pay for
and receive the services they need. It also does not penalize residents that do
not require additional services to pay for something they do not need.
Quantitative Assessment of Resident Care Needs is completed by the Health
Services Coordinator (a licensed nurse) and will be discussed with residents and
responsible parties prior to any additional charges being assessed. Optional
services and charges require the signed approval of the responsible party and
may include any of the following activities of daily living:
- Bathing
- Dressing
- Continence
- Ambulation
- Nutrition
- Medication Management and Distribution
- Cognition
- Lifestyle Management
- Complexity of Care
